无尘室施工安全卫生作业规定

1. 施工前:
1.1 施工申请
1.1.1 承包商应于施工前填写【开工报告单】、【工作场所环境及危害因素通知书】、【无尘室施工申请表】,若有特殊作业须填写特殊作业申请表单(【动火作业申请表】、【夜间作业申请表】),所有表单交由环安卫部门确认。若有用电需要须填写【临时用电申请表】由厂务部进行核准。
1.1.2 进入无尘室申请
承包商由厂务部代为申请【无尘室进入申请单】。
1.2 紧急施工的申请
若有紧急施工或维修的情况,且进入的人员亦未经本公司工安教育训练时,发包单位应负责告知施工场所安全卫生作业规定及危害注意事项并指派监工人员,经制造主管核准后方可进入。

2 承包商进入无尘室须遵守之规定
2.1 所携入的物品、器具合乎无尘室要求。
2.1.1 除原物料、大型机台(或零组件)、大型器具(如工作桌)无法由洗尘室进入无尘室外,其余物品与工具等,均应以无尘布沾5%酒精擦拭后,才可进入空气洗尘室,携入无尘室。
2.1.2 进入无尘室的大型器具、大型机台(或零组件)等,均须在缓冲区将包装材拆除,并以5%酒精擦拭干净,始可搬入。
2.2 承包商进入无尘室需有相关人员陪同。并登记在【无尘室进出管制表】。
2.3 不可化妆,包含口红、粉底、指甲油、眼影、香水。个人应保持良好卫生习惯。
2.4 不可携带私人物品,如香烟、打火机、食品、化妆品、手镯、耳环、金戒指、手机等。
2.5 除无尘室专用文具外,严禁携入铅笔、自来水笔、挥发性笔、一般性用纸等。
2.6 一般衣物或工程安全帽应放置于员工休息的挂衣架、内务柜或办公室,不可挂于更衣室或无尘鞋柜。
2.7 若有吸烟则30分钟后方可进入无尘室。
2.8 不得穿着员工的无尘衣物。
2.9 人员进入无尘室时,需遵守无尘衣的穿著规定,依据各无尘室Class等级穿着规定的等级无尘衣、头罩、鞋子及口罩。
2.10 禁止搬入木质用具或易生粉尘材质的器具。

3. 施工期间:
3.1 施工人员进出工作地点,务必配戴工作识别证和承包商环安卫训练合格证。
3.2 禁止用推车顶门及用工具、脚接触、踩或踢门。
3.2 机台的管线应用束线带束紧,固定于墙角或固定在不妨碍通行的角落。
3.3 施工时有电线的部份,应使之不散落或不交错放置走道上。不使用胶带固定电线于地上或墙上且不论何种电线均不可用脚踩踏。
3.4 使用电源时须使用厂务电工指定之插座或电盘,且要确定所使用之工具的电压与插座电压是否相同。
3.5 严禁切削工程,如有必要,需先作防护措施(以真空吸尘器、塑料布铺于地板上)方可进行。
3.6 施工场所仅能存放当天施工所需之物料。
3.7 施工场所,应以隔板或施工警示带隔离施工区且应设有标示〝施工中〞之告示牌。
3.8 施工中需用之化学品或气体,必须于事前征得本厂监工人员同意才可使用(如有需要,可要求本厂环安卫部门协助)。
3.9 严禁吸烟、饮食。
3.10 任何可能触动消防系统或需开启安全门之作业,应事前由监工单位会相关单位同意,并告知环安卫部门。
3.11 不清楚的化学药品绝不可自作主张、擅自处理,以免危险之发生。
3.12 施工中严禁攀爬、踩踏机台设施,严禁攀扯任何气体/纯水管件,如因违反规定影响生产造成意外及损失,由承包商负责赔偿。
3.13 如有可能影响本厂环境之作业(如油漆或施工过程中会产生化学品气味),应事前告知监工人员、影响区域之主管及环安卫人员。
3.14 任何可能影响生产作业(如停电、水、气等)者,必须事前经本厂监工单位及现场负责人同意。如未事前知会造成生产中断损失,由承包商负责赔偿。
3.15 作业前务必清楚工作区域各式管件、储槽内之物品种类及其危害性。
3.16 作业中应使用堪用、安全之工作梯或工作台进行高处作业。
3.17 作业过程中,可能接触任何危害物质时,应穿戴正确且有效防护具。
3.18 掀开高架地板施工,应立即设置警告围篱,休息、下工或完工时,必须将高架地板复原后,始得离开作业区。
3.19 工作期间若发生火警或化学品泄漏等意外事故,施工者应视情况进行紧急处理若情况严重须立即停止工作,离开危险区域,并通知监工人员进行事故通报。

4. 施工完毕后:
4.1 承包商于每日施工完毕后,须运走现场所有之垃圾且待监工人员至施工现场检查,并填
写【承包商施工日检表】,合格后承包商方可凭此表单离厂。
4.2 工程结束后承包商须交回环安卫训练合格证及所有申请表单。

1        PURPOSE 目的
The purpose of this standard is to ensure management, staff and all employees receive suitable and adequate information, instruction and training in the meaning of safety signs and the measures to be taken in connection with them 编制本标准主要是为了确保管理者、雇员能够正确并充分地了解安全标识的信息、指令和学习相关知识,以便据此采取相应的措施。
2        SCOPE 范围
This procedure covers the set up and maintenance of signs and barriers. 本程序概述了标识和隔离的建立和维护。
Pictograms or graphic symbols may help to describe a situation, to prescribe a determined attitude or to indicate a danger. 图形和图解符号可有助于说明状态,指明一个明确的态度或指明一种危险。
Barricades are placed on the job site to restrict entry and/or provide warning for areas that involve construction activity, that contain safety hazards, abnormal conditions, or in which unusual operations are being performed. 在工作现场建立围护,防止无关人员进入和/或警告该围护区的施工作业存在安全危险、异常情况或正在进行特殊作业。
3        DEFINITIONS 定义

4        RESPONSIBILITIES 责任
        Site manager is responsible for ensuring that this procedure is issued and implemented. 现场经理负责确保程序的发布和实施。
        Employees are responsible for ensuring themselves following the safety signs. 各员工负责遵守安全标识。
5        PROCEDURE 程序
5.1        Safety signals 安全信号
Rules for using safety signals are fixed by laws: 根据法律法规的要求使用安全信号:
        Panels, colors, light and sound signals may be used for safety and security signaling.标识牌、颜色、灯和声音信号可用做安全和保卫信号
•        It is always requested if a risk cannot be prevented or avoided by a collective protection or by a proper working organization. 如果不能通过统一防护或一家合理工作组织规避风险,可以一直使用安全信号。
•        Pictograms must be as simple as possible without useless details. 图案应尽可能简单,没有多余的细节。
•        Pictograms must be as big as possible to be quite visible. 图案必须尽可能大,以便醒目易辨。
•        Pictograms may be slightly adapted or detailed only if the message provided has not been modified. 如果提供的信息不能完全达意,可以适当变动或详细化。

It’s up to the Site Manager to determine signs to be installed or to be used according to identified risks. 现场经理有权决定安全标识的竖立,或根据确定的危险类别使用。
Company, Contractor, Subcontractor, vendors and visitors people must be informed and trained as often as necessary on safety colors and panels signification. 必须经常告知公司、承包商、分包商、供方以及访客安全色和标识牌的内容,并进行相应培训。
Panels’ signification depends on the shape, the color and the pictogram used: 标识牌的内容主要取决于形状、颜色以及使用的图案。
•        Prohibition sign: prohibition sign concerns dangerous behavior. It must be round shaped with a black pictogram on white background with red edging and a red diagonal line (top left, bottom right). The red part must take up at least 35 % of the panels surface). 禁止标识:禁止标识指明危险行为。必须使用圆形、黑色图案、白底、红边和一条红色的对角线(左上、右下)予以表示。红色部分必须至少占标识牌面积的35%。
Example: 示例:

•        Warning sign: warning sign concerns the need to be careful, take precautions, examine or the like (e.g. the safety color on hazard signs, such as flammable material, electrical danger, etc.). It must be triangle shaped with black pictogram on yellow background with black edging (yellow must be at least 50 % of the panels surface). 警告标识:警告标识表明需要小心、注意以及需要检查等信息(比如危险标识上的安全色,如易燃材料、用电危险等)。必须采用三角形、黑色图案、黄色背景、黑边(黄色必须占标识牌面积的50%)。

•        Mandatory sign: mandatory sign requires specific behaviors or action. Mandatory signs must be round shaped with a white pictogram on blue background (blue must be at least 50 % of the panels surface). 强制性标识:强制性标识是对行为或行动的明确要求。该标识须采用圆形,白色图案、蓝底(蓝色必须至少占标识牌面积的50%)。
Example:示例:  

•        Escape and help sign: escape and help sign must be square or rectangle shaped with white pictogram on green background (green must be at least 50 % of the panels’ surface). Guidelines for preparing health, safety and environmental plans

Introduction
Prior to the commencement of the contract, the contractor may be required to submit a Health, safety and environmental management plan (HSE plan) detailing the systems and procedures which will apply during the term of the contract.
The HSE plan is to be completed and submitted to the contract administrator within 14 days of contract execution.  No work is to commence under the contract until ActewAGL approves the HSE plan.
The HSE plan will be reviewed and be subject to approval by ActewAGL.
The HSE plan shall be reviewed at regular intervals throughout the contract to ensure that it is maintained in an up-to-date condition.  The contractor’s HSE plan will also form the basis by which its management systems will be audited by ActewAGL.
This document provides guidance to contractors when preparing their plan, by outlining the general requirements and elements of HSE plans.
Contract description
A brief description of the scope of work associated with the contract should be documented.  The description of the contract should be sufficiently detailed to provide persons unfamiliar with the contract an overview of the type of work being carried out and under what conditions.
The scope of work should include, the following details:
o        summary of major activities and types of work to be performed
o        list of tasks or specialist procedures that may require detailed environmental review together with health and safety work procedures and training
o        list areas of contract requiring special consideration from a HSE perspective including:
o        presence of public
o        traffic management
o        disturbance of soil, location of trees
o        work restrictions (work-times, confined spaces)
o        exposure to hazards (noise, dust, elevated heights)
o        specific local government requirements.

Contract HSE structure and system
The contractor’s HSE plan should be established around existing HSE management systems and associated procedures and controls.  Reference should be made to existing procedures and documentation in the HSE plan.  This will also assist in minimising the size of the document.
The contractor should outline the management structure, responsibilities, standards and control systems applicable to the contract to ensure HSE requirements are adequately addressed.  The following information should be included:
o        company HSE policy, to be displayed at work-sites





o        an outline of the contract HSE organisation and structure, such as names and/or positions of those with specific HSE responsibilities
o        summary of HSE roles and responsibilities of contractor staff involved in the contract
o        position and/or name of senior person who will liaise with ActewAGL on HSE issues.

Contract HSE induction and training
OHS and environmental legislation requires all employers to ensure that their employees have the skills and training required to carry out their work in a safe manner and without negative impact to the environment.  ActewAGL requires that contractors document their HSE training programs ensuring that they have appropriately skilled employees, suitable training programs and adequate supervision for the contract works.
The following information should be provided:
o        an outline of contract HSE induction procedures for employees and sub-contractors
o        details of HSE induction course content
o        register of personnel who have satisfactorily completed the Contract HSE induction
o        details of employee HSE training that has or will be provided relevant to the contract requirements
o        provide a register of names and/or positions of contract employees with authorisations, permits, competency certificates, licences and the like, who may be required to supervise or undertake specialist work activity.

HSE work practices, work method statements and procedures
Where appropriate, relevant HSE work practices and procedures/work method statements should be developed for the contract.  Where possible, existing HSE company procedures should be used.  However, contract specific HSE work method statements and procedures may need to be developed on the basis of particular contract hazards.  These may be identified when undertaking the risk assessment.
The following information should be provided:
o        a list and copies of company HSE work method statements, procedures or instructions relevant to the contract
o        a list and copies of contract specific HSE work method statements, procedures or instructions
o        details of site operations, which will be subject to Permit to work systems
o        details of employees and/or sub-contractors issued with copies of HSE work method statements, procedures and instructions
o        HSE work method statements, procedures and instructions should be recorded in a Safe work procedure or instruction register.

Risk assessment
The risk assessment is an integral part of the HSE plan and considers the following:
o        identifies hazards associated with contract tasks and activities
o        determines the level of risk
o        establishes appropriate risk control measures.




Each major or significant task or activity associated with the contract shall be assessed in terms of the associated hazards.  When all hazards have been identified the most likely outcome as a result of an incident shall be determined.
Risks shall be classified according to the potential consequences:
Catastrophic        Multiple fatalities, toxic release off site with detrimental effect, financial loss over $10,000,000
Major        Extensive injuries to staff or public or single fatality caused, loss of operating capacity, any environmental releases off-site with no detrimental effects, any damages or losses are limited between $1,000,000 and $10,000,000
Moderate        Medical treatment is required to staff or public, any environmental on site releases are contained with outside assistance, any damages or losses are limited between $100,000 and $1,000,000
Minor        First aid treatment may be required to staff or public, any environmental on site releases are contained locally, any damages or losses are limited between $10,000 and $100,000
Insignificant        No injuries to staff or public, any damages or losses are limited below $10,000
A primary goal shall be to eliminate potential moderate, high or catastrophic risks associated with the contract and this should be a major focus of the risk assessment.  Contractors should detail risk control measures, which adequately address all such risks.  When determining risk control strategies, the hierarchy of controls summarised below should be considered:
Hierarchy of controls        Examples of health and safety controls        Examples of environmental controls
Eliminate the hazard
        Undertake cleaning without need to enter confined space         Define no access zones around environmentally sensitive areas within the work site
Substitute the hazard
        Replace toxic substance with less harmful substance        Use of biodegradable products instead of non-biodegradable products
Engineering controls
        Install ventilation system to remove hazardous atmosphere        Use of prefabricated access equipment for crossing watercourses or in boggy areas
Administrative controls
        Permit to work systems and associated training        Environmental authorisations or processes and associated training
Personal protective equipment
        Respiratory protective devices        Tree protector, erosion and sediment control devices
Where HSE work procedures or instructions are developed, they must clearly spell out the work sequence, highlighting the procedures required to adequately control each moderate, high or catastrophic risk identified in the risk assessment.  All employees involved in the activity shall receive appropriate training in the HSE work procedure.
The risk assessment shall be completed on the Risk assessment form (provide link) evaluating the full scope of work associated with the contract.  Additional risk assessments


may be undertaken during the course of the contract as required, that is work undertaken by sub-contractors.
Workplace HSE inspections
HSE inspections play an important role in the identification of hazards at the workplace and in the development of control measures.  The HSE plan should outline the procedures and methods by which contract workplaces will be inspected on a regular basis.
The following information should be provided:
o        details of how workplace HSE inspections will be undertaken during the contract, considering:
o        checklists to be used
o        frequency of inspections
o        team members
o        who is responsible for reviewing HSE inspections and ensuring action is taken to address HSE risks
o        details of plant and equipment maintenance records
o        details of hazard reporting procedures for the Contract, including hazard report forms
o        details of specific activities or areas targeted for inspection including plant, hazardous materials, electrical safety, oil management, sediment controls and emergency equipment.

HSE consultation
Consultation with employees provides an important mechanism whereby HSE issues can be dealt with in a manner that promotes ownership and prompt resolution.
The following information should be documented:
o        list of current employer and employee Health and Safety representatives
o        details of the membership and operation of a HSE Committee
o        reference to company issue resolution procedures.

Emergency procedures
There is the potential for a range of emergency situations to occur both on-site and off-site in relation to Contract works.  These situations need to be identified and specific emergency procedures developed and made known.
The following information should be documented:
o        overall emergency plan and structure for the contract
o        register of emergency equipment and locations, such as first aid equipment, fire extinguishers, spill kits
