为贯彻落实政府关于职业健康安全和环境管理的法律、法规、规章,强化公司安全环境管理,杜绝死亡、交通事故、重大火灾及重大环境污染事故,严格控制重伤事故,抑制一般事故的发生,确保公司生产安全有序,特制订本《安全责任承包合同书》(以下简称《合同书》)。
In order to implement government’s laws, regulations and rules about professional health and safety and environmental management, strengthen safety environment management, avoid death, traffic accidents, big fire and severe environmental pollution accidents, strictly control severe injury, contain ordinary accidents and ensure the safety and order of production, hereby we specially establish this “Safety Responsibility Contract”(referred as to “Contract” in short hereinafter)
一、        责任:  
I.        Responsibility
各部门正职是属地安全/环境控制第一责任人,管辖区域内安全生产、环境保护的考核指标对总经理负责。
Managers of all departments are the first person responsible for the safety/environmental control of their field and report directly to GM about safe production and environmental protection in their jurisdiction.
二、        期限:二0一0年一月一日至二0一0年十二月三十一日

五、安全/环境管理主要内容:
V.   Main Content of Safety/Environment Management:
1、各部门负责人应根据《合同书》中《事故控制指标》、《基础管理目标》的要求进行分解,制定出适合本部门的《安全生产责任分级承包合同书》或《安全承诺书》,并在组织员工学习后签订。
   All depts. should break down according to requirements in the “accident control index” and “basic management target” of “Contract”, establish “classified safety production responsibility contract” or “letter of safety commitment”, then organize personnel to sign after studying.
2、对于车间,要求每周组织一次安全环境自查整改工作,并做好相应的检查整改记录;每月二次的班组安全学习活动由车间根据实际生产情况,自主组织安排好每月二次的班组安全学习活动,并做好学习记录。
  A weekly self-check of safety environment is mandatory for workshops and relative inspection and correction records must be kept; safety study by shift twice every month should be organized according to actual production in the workshops and study records should be kept.
3、认真做好事故隐患整改和危险源监控工作。对公司发出的《隐患整改通知单》要积极采取措施,在规定的限期内组织、协调、落实整改;对因技术、资金原因,一时难以整改到位的必须采取有效的临时措施,并向员工告知,确保安全生产。
Manage the correction of potential risks and monitoring of danger source, take active measures for “Correction Notice of Potential Risks” from company, organize, coordinate and implement before the deadline; take effective temporary actions for open points that cannot be closed totally in a short period and inform the workers to ensure safe production.
4、确保各岗位员工在上岗前得到足够的安全培训。铲车工、维修电工、配电值班工、空压工、电焊工、行车工等特种作业人员必须持证上岗。
  Ensure all personnel to have enough safety trainings before starting operation. Special operation personnel like forklift drivers, electric maintenance worker, worker in the power room, air compressor operator, welding worker, travel car operator cannot operate without a license.
5、坚持安全生产“五同时”,即在计划、布置、检查、总结、评比生产工作的同时,同时计划、布置、检查、总结、评比安全工作,并做好相关的安全工作记录。
   Adhere to the principle of “plan, implement, check, conclude and evaluate at the same time” and keep relative safety records.
6、各车间及相关部门应关注垃圾分类工作;禁止向地表或下水管沟倾倒废油、废切削液、试验用的废酸碱溶液等,此类废弃物应使用专用容器盛装,集中后存放于厂区南面的专业垃圾槽内,由制造科统一处理;试验室用完的,盛装硝酸的容器,应作为危险废弃物存放厂区南面垃圾场,严禁随意丢弃或移动它用。
  All workshops and relative departments should pay attention on refuse classification; forbid to pour waste oil, cooling liquid and waste chemical acid and alkali from experiment to the floor or sewer system. They should be kept in special container and stored in the special refuse pool in the south of plant and handled by manufacturing department; container after containing nitric acid from the lab should stored in the refuse area in the south of plant, arbitrary dumping or moving of them is forbidden.
7、 严格事故报告与调查处理制度。安全/环境事故发生后,责任部门负责人应迅速采取有效措施,并在第一时间组织抢救,防止事态扩大;;保护好事故现场,并按有关规定和程序立即上报;按照“四不放过”的原则,查明事故原因、落实整改措施、分析事故责任并提出对事故责任人的处理意见。
   Make a strict accident report and investigation system. After safety/environment accidents happen, managers of department responsible should take quick and effective measures and organize the fixing in the very first time to prevent it from escalating; protect the accident scene and immediately report to supervisors according to relative protocol; find out the reason, implement correction measure, analyze the responsibility and propose how to do with the personnel responsible according to the principle.
8、依据《三同时管理程序》,技术科、设备科、行政人事部、制造科等相关部门应高度关注新、改、扩建项目以及新设备、新材料、新工艺、新技术项目规划、设计、施工、生产和使用过程中的职业安全、卫生、环境保护、消防“三同时”工作并归口负责。职业安全、卫生、环境保护、消防投资应当纳入建设项目概算(预算)。
      Relative department like process, maintenance, HR and manufacturing should pay high attention on professional safety, health and environment protection and fire fighting in the planning, design, construction, production and use of new, modified and extended project and new equipment, materials, process and technical projects and will be responsible. Investment of professional safety, health, sanitation, environmental protection and fire fighting should be considered into the budget of projects under construction.
9、加强特种设备和主要生产设备的安全管理,按规定进行经常性维护、检修及定期检测,确保安全防护装置齐全有效。
   Strengthen safety management of special equipment and main production equipment, carry out usual maintenance, repair and calibration and ensure safety protection device is complete and effective.
10、完成甲方下达的其他评价或评审指标。
   Finish other evaluation and audit index from Party A.
六、考核与费用管理
VI. Evaluation and Cost Management
1、        甲方依据《安全奖惩管理考核办法》对各部门上述指标的完成情况进行月度考核,其中本年度对发生轻伤事故的单位考核以以下描述为准:公司月度轻伤事故控制数为1起,年度轻伤事故控制指标为2起,指标分解为制动盘车间1起、机加工车间1起,其余部门为0起。
Party A will monthly evaluate the performance of all departments on above mentioned indexes according to “Safety Reward and Punishment Evaluation Regulation” in which the target of minor injury in 2010 is as follows: minor injury must be no more than 1 in one month and no more than 2 in one year and there should no more than 1 in disc workshop and the same for machining workshop. For other departments, there should be no minor injuries.
①在指标控制数内发生的轻伤事故,对责任部门第一责任人处罚人民币1000元/起;
  If the quantity of minor injury is under the target, the punishment for person firstly responsible will be 1000 RMB every time;
②在指标控制数外发生的轻伤事故,对责任部门第一责任人处罚人民币5000元/起。
  If the quantity of minor injury is over the target, the punishment for person firstly responsible will be 5000 RMB every time;
2、        对事故考核指标超标部门,实行否决制度。
Veto by one vote system will be carried out on the department in which the quantity of accidents is over the target in one department
3、        安全处罚费用由财务部按规定设立“安全专项资金”,统一管理,主要用于表彰对公司安全/环境管理工作有功人员和部门,以及组织安全管理活动、隐患整改等。
Punishment company charges will be taken to establish a “special safety fund” by financial department mainly to reward personnel and department with contributions to company safety/environment management and organize safety management activity and correction of potential risks.
七、其它
VII. Others
本《合同书》经甲、乙双方签字后立即生效。甲、乙双方责任人如有变更,本《合同书》相关考核指标对新上任的部门负责人具有连续承前的约束效能。
This “Contract” will take effect immediately after signing of both parties. Should there be any change of people responsible, relative evaluation indexes of this “Contract” are consistently binding for the new person responsible.
