工 作许可

Purpose目的

This procedure is to assist the owner and work groups of an area to assure that area’s conditions are safe to proceed with planned work and that all safety considerations have been adequately addressed.
本程序旨在帮助区域业主及工作组确认该区域的工作环境是否安全，可以按计划执行工作，并充分考虑到涉及的所有安全问题。

General通则
2.1
A work permit will be used for all work done in an area, unless the owner and his workforce do the work.
If the owner does the work, a work permit is optional.
除非业主或其工作组从事该项工作时可任意选择，任何人执行区域工作都必须使用工作许可。

2.2
Some routine and frequent jobs may be excluded from this requirement if a complete written procedure exists.
对一些日常及常规性工作，若有完整的书面程序存在，则可不包括在本程序要求之内。

2.3
Work permit form consists of two sections. The upper section is designed to show work description and the lower section show the safety preparations required for the job.
工作许可由两部分组成。上部分用以描述工作内容，下部分则说明执行该项工作所需的安全准备。

Procedure程序

3.1
Responsibility
职责

The work group supervisor in charge of the work will initiate work permit. The permit will be approved and signed by responsible supervision in the area where the work will be performed.
工作许可由负责该项工作的工作组主管填写，并应得到该工作执行区域的主管负责人的签署及核准。

It is a joint effort and responsibility between work group supervisor and owner supervisor to fill out work permit properly.
First, work group supervisor fills in the work description section and checks safety preparation items of the permit before taking it to the owner.
The owner supervisor needs to review and complete the permit, including specific permit requirement and other safety procedures.
正确填写工作许可是工作组主管及业主主管共同的责任。首先，在提交业主主管之前，工作组主管应填好工作描述部分并检查安全准备条款。然后业主将对此工作许可予以审查并完成其余事项，包括一些特别许可要求和其它安全程序。

The owner must indicate required preparation of equipment for work to be performed. The owner must identify known or suspected contaminates and indicate classification and protection need.
业主必须说明执行该工作所需的设备准备，辨识可知或可疑的危害源并指明类别及必要的防护。

Supervision of work group must note any additional requirements necessary to perform the work safely and use the permit as a checklist.
工作组主管必须注明其它一些必要的安全要求，并将工作许可作为一份检查表使用。

3.2
Work Description Section
工作描述部分


Work description section of the permit shall be completed as outlined.
Specific description of the work should be written on the permit.
The following should be addressed in filling out the section of the permit.
对工作许可中的工作描述部分进行扼要说明，明确描述工作内容。在填写该部分内容时应注意以下事项：


a.
The Work Permit will define the location of the work so that the owner and work group can understand the work area well enough to clearly identify any hazards.
工作许可应说明工作地点，以便业主及工作组充分了解工作区域，从而辨识所有危害。

b.
The Work Permit will provide enough details of the work so that the owner can determine what hazards or impact on operations may occur while the work is being performed.
工作许可应提供工作的充分细节，以便业主确定在工作执行中对操作产生的危害或影响。

c.
Specify any hazardous conditions or hazardous material.
确定所有危害情况和危害性物质。


d.
The Work Permit can be filled out ahead of time but can only be signed on the shift the work is to be performed.
It should be signed as close as possible to the start of the job after owner completed the preparation and lockout.
This will help the Area owner to co-ordinate activities and prevent exposing workers to hazardous situations.
可提前填写工作许可，但必须在工作执行的班次方可签署。工作许可应在业主完成准备及上锁后尽可能接近工作开始时签署，这将有助于区域业主进行协调工作以防止员工暴露在危险情形下。


3.3
Safety Preparation Section
安全准备部分

The following considerations to be given to filling out the Safety Preparations section. All section’s pertinent conditions known or suspected on each job requiring a permit shall be completed.
填写安全准备部分应考虑以下事项。所有与需要许可的工作的可知或可疑的相关的条件均应填妥。

3.4
Authorized Work Permit Form
授权工作许可

a.
The work permit is valid for 3 days with the following conditions adhered to:
下列情形下，工作许可有效期为三天：

-
Work permit reviewed by all on-coming shifts.
经每一班次审查。
-
All conditions of the original authorization are the same with no exceptions.
最初授权的工作许可条件没有任何变动。
-
Re-authorized each 24 hrs by the owner supervisor and work group supervisor.
业主主管及工作组主管每24小时要重新授权一次。

If any condition has changed, a new permit must be issued to continue the job.
若条件有变动，则应签发新的工作许可。


b.
Work group shall post one copy of authorized permit at the job site when job proceeds and be removed once the job is completed or stopped.
工作组将授权许可张帖在工作现场，并在工作结束后取走。

c.
Another copy of authorized permit shall be copied to area owner.
业主保留一份授权工作许可的复印件。

3.5
Contract Administration
承包商管理

When a work permit is issued to a contractor, the responsible contractor co-ordinator will initiate the work permit.
当一项工作许可签发给承包商时，由负责承包商的协调人员填写工作许可。

The contractor’s on-site supervisor must understand by reviewing it with responsible contractor coordinator and experienced area personnel and then approve the precautions by signing the permit.
承包商的现场主管应通过负责承包商的协调人员及有经验的区域员工对工作许可进行审查并签署同意特别小心事项。
