his employer should provide FRC(fire resistant clothes) to him,and I think that the root cause analysis reports is bad,if you are a professional safety engineer,you shouldn't blame the the operator so much,particularly,Operator's behavior is not the sole cause of this accident.

we need ask ourselves why the TA was silly, do we teach him the standard operation process effeciently. if all operators could be talented enough, what is our value? to help them to work safely, that is our mission.
proper standing pose could avoid this risk,
blanket is what i think also essential, ignition source should be isolated away from combustible substance. this is another scenario for you to control the ignition source.
