山西好像在搞百日督察活动，恰恰在此间出现了尾矿溃坝事故。

So many severe incidents occured in short time, it shows our safety management system has problems. As the article in the forum described: safety management system implemented not fully put in practice; safety awareness not be past to all the works, and preventative measures not be enough arranged;as well as no enforced supervisor for the high risk tasks.
