13.0
EMPLOYEE SAFETY ORIENTATION/TRAINING

All employees, on their initial assignment to the Fluor Daniel/BP Pasadena 15 Project, shall attend the BP general and site-specific orientation conducted by the Houston Area Contractors Safety Council and the Fluor Daniel/BP Pasadena 15 Project new hire orientation program conducted by the Fluor Daniel Site Safety Department. Employees must successfully pass written exams.

Fluor Daniel/BP Pasadena 15 Project Site Management will participate in new hire orientation to share project goals and company safety culture/expectations with all personnel.

Employees required to enter operating units shall attend a separate unit specific orientation (Process Overview).

§ Scaffolding

§ Portable ladders

§ Mobile equipment

§ Cranes

§ Excavation and trenching

§ Rigging

§ Hazard communication - Requirement for - Container labeling - Material Safety Data Sheets

Continuous training will be provided to employees by means of daily Safety Task Assignments “ STA” communications, weekly supervisor’s meetings and employee tool box meetings.


Formal training will be provided to applicable employees assigned to perform specific tasks:

§ Trenching and excavation

§ Confined space, entrant, competent person

§ Fire watch

§ Respiratory protection

§ Permit acceptor

§ Process overviews

§ Chemical hazard recognition

§ Initial line breaking

§ Pressure testing

§ Infectious disease

§ Lead abatement

§ Scaffold construction

§ Scaffold inspection and use

§ Benzene Reduction

§ Hearing Conservation

§ HAZWOPER


In addition to the above mentioned programs, the Fluor Daniel Corporate Safety Department will be available to conduct training courses on Trenching and Excavations, Process Safety Management, Safety Leadership, Substance Abuse etc.

13.2
Training Schedule

The Fluor Daniel/BP Pasadena 15 Project Safety Department will develop a training schedule. This will identify the training programs to be presented and the frequency of refresher training. In order to assure comprehension of the material presented, competency testing will be conducted.

14.0
PROJECT ASSESSMENTS/AUDITS

A formal assessment of work area(s) will be conducted each week. The weekly assessment will include contractors associated with the project.  A member or members from each assessed group will be expected to participate in the assessment of their respective area(s). Participants of the assessment will include, as a minimum, a safety representative, general foreman and or foreman.   

The findings, corrective actions required and responsible supervisor will be documented on the project assessment report form.  A copy of the assessment report will be issued to the Fluor Daniel Site Construction Manager, responsible supervisor and or Subcontractor’s Site Manager by the end of the shift.  In order for the loop to be closed, the supervisor responsible for corrective action(s) will turn the assessment form into the Fluor Daniel/BP Pasadena 15 Project Site Safety Department with in 24 hr.  The supervisor shall indicate corrective action complete or incomplete.  If indicated as incomplete, a status report will be attached.  Outstanding items will be documented as such on the following week’s assessment report.

The weekly assessment will be scored based on points possible, applicable to the work being performed, and points awarded based on the findings.

Project management will track the supervisor’s performance weekly and monthly. In the event the supervisor’s score is 85 or less a meeting will be scheduled with project management to assist in identifying and correcting the reason(s) for the unsatisfactory performance.

A supervisor scoring less than Eighty (80%) shall cease work in the respective area and commence corrective action immediately. The respective superintendent or manager shall provide a written plan of action as to steps that will be implemented to improve the safety performance.  The plan shall be submitted to Project Management for review and approval within twenty-four (24) hours of the assessment.

14.1
Corporate Assessment

Quarterly, a member of the Fluor Daniel Corporate Safety Department will conduct detailed formal audit.  The project will be scored based on audit findings.  The score and findings will be distributed to all project management and the Director of Corporate Safety.  In order to close the loop, the Fluor Daniel/ BP Pasadena 15 Project Construction Manager will be required to respond in writing listing corrective action.

The Corporate Audit consists of a twenty-seven-page document divided in two sections, Administrative and Field Execution.

The Administrative Section consist of sixteen sub-sections that reviews in detail the implementation and document of mandatory programs i.e., hazard communication, medical records, inspection programs, operator certifications, confined space, infectious diseases, etc.

The Field Execution consist of twenty-three sub-sections that review the safe execution of field activities i.e., welding and burning, electrical, steel erection, excavating and trenching, power tools and power equipment, etc.

14.2 Safety Recognition/Incentive


Incentive: A proposed monetary incentive will be paid for project safety performance. The safety performance will be tied to weekly Safety Assessments. Eligibility for incentive will be contingent on scoring 95% or better.


Special Recognition Safety Coupons:  This award is a coupon issued by the Fluor Daniel/BP Pasadena 15 Project Safety Department Members, only to employees in special recognition. Any employee that demonstrates the ability to recognize any Safety problem that could have been fatal, cause serious injury or major equipment damage will receive this coupon and redeem it in the Safety Office for the employees choice of a Zero Accident Logo awards.



Project Lunches:  Held at each recognizable event for all employees at 100,000, 250,000, 500,000, 750,000 and 1,000,000 Safe Work Hour Accomplishments. Lunches will be based on no LTI’s and TRIR of <0.8.
