One Lock, One Key, OnePerson: The basis of LOTO is that any individual has “total control” of thelockout of the machine, equipment, process or circuit that is being servicedand/or maintained. This concept assures the individual sole lockoutresponsibilities. “一人，一锁，一钥匙”：只有这样才能确保授权者对阀门、开关的绝对控制。 2. Effective Training: Understanding lockout responsibilities, processes, methods andrequirements result from effective LOTO training practices. The knowledgeattained through training is reflected by practices of the site/operation.Effective training is proven by periodic inspection and includes provisions fordemonstration and performance feedback. “有效培训”：不仅要培训，还要检查培训的效果，随时跟进。 3. Alternative Methods: Complete LOTO isalways the first choice. Alternative methods are only permitted if 1) LOTO isnot feasibleor 2) the activity is considered to be routine, repetitive and integral to theproduction process. The alternative methods must be developed through RiskAssessment that includes identification, review and communication of additionalregulatory requirements such that adequate protection is provided. “其他方法”：LOTO是首选，但未必完全适用，选择合适的方法才是正理。 “授权人员和受影响人员”：要通过培训让管理人员和一般工人对LOTO的理解达到相应的程度。 5. The Proper Tools: LOTO is conducted with aspecific set of tools. These tools include keys, locks, multiple lockoutdevices, red tags and transitiontags. Proper use and application of these tools is learned through thisprogram and effective training. “合适的工具”：合理使用工具，其实brandy等老美的工具还是挺坑钱的。 6. Equipment-specific LOTOProcedures: Equipment-specific LOTO procedures provide detailed guidelines for safelockout of each machine, equipment, process or circuit that is being servicedand/or maintained. “具体到单个设备的LOTO程序”：更细化的管理，具体到单个设备，相当于操作规程了吧？ 7. LOTO Verification: A “Zero Energy State” mustexist in the machine, equipment, process or circuit before servicing and/ormaintenance activities can begin. The best method possible must be used toverify that energy has stopped, released, dissipatedor drained completely. Verification provides certainty that energization cannotoccur while work is being performed. Theverification (or “tryout”) step must be included in each equipment orprocess-specific LOTO procedure. “LOTO确认”：必须要有一个验证“零能量”的步骤。 8. Shift or Personnel Changes: The maximum permittedduration for a LOTO is one shift or the end of the task, whichever isshorter. Continuity of the LOTO processmust occur as shifts or personnel change. This applies to both individual andgroup LOTO. It is important to assure that the LOTO process remains intactduring these transition times by using direct LOTO hand-off, TransitionLocks or other approved methods. “交接班和人事调动”：交接工作时要交安全，当然包括LOTO。 9. LOTO for ContractActivities: Whether contractors are working at asite/operation or General Electric individuals are performing contract work, itis of the utmost importance that all aspects of the General Electric LOTOpolicy are adhered to. As a best practice, the General Electric authorizedrepresentative may perform the LOTO step-by-step process (refer to Paragraph 5.2.1). Theoutside service or contractor will then be required to attach and secure IndividualLOTO Locks and red tags to the same energy-isolating devices that theGeneral Electric representative has locked out. This is commonly called"GE first on / last off". “承包、发包的LOTO管理”：不管是作为业主还是承包方，在这个现场中所有各方都要使用GE LOTO。 10. Risk Assessment: Risk assessment explores thesafest conditions possible for individual work assignments. Risk assessmentestablishes safe practices and alternativemethods to reduce the possibility of injury when normal LOTO procedurescannot be applied. The risk assessmentmust also include identification and implementation of control measures forother regulatory requirements. “风险评估”：HSE管理的本质就是风险管理。 |
